Responsibilities - Conduct comprehensive assessments of residents' physical, and psychosocial needs - Ensure accurate and timely completion of Minimum Data Set (MDS) assessments - Collaborate with the interdisciplinary team to develop individualized care plans based on residents' needs and preferences - Coordinate and communicate with healthcare professionals, residents, and their families regarding the care plan - Provide education and guidance to the nursing staff on proper care plan implementation - Stay updated with regulatory guidelines and ensure compliance with all applicable regulations and standards Requirements - Current nursing license in Illinois - Ability to work effectively as part of a collaborative team - Compassionate and patient-centered approach to care - Excellent communication and interpersonal skills - Long-term care, MDS and Care Plan experience preferred, but not required Job Type:
